California Healthy Kids Survey

Dear BBHS/CTHS Parents and Guardians,
Your child will be asked to participate in our school’s California Healthy Kids Survey (CHKS), sponsored by the California Department of Education (CDE). This survey helps promote student health and well-being, improve the school environment, and address concerns such as school safety, health behaviors, and alcohol and other drug use.
The survey includes questions about school connectedness and supports, barriers to learning, physical activity and nutrition, alcohol, tobacco and other drug use, risk of depression and suicide, and protected class identifiers such as sexual orientation and gender identity.
Participation is voluntary. Students who, with your permission (and signed opt out form), may skip any question they do not wish to answer and may stop at any time. The survey is anonymous and no names are recorded or attached to responses, and results are analyzed only under strict confidentiality controls.
The survey will be administered between March 13 and April 3. You may examine the questionnaire in the school office or at your district’s Web site

If you would like your child not to participate, please contact the BBHS office to request an opt-out form. Completed forms must be returned to the school prior to March 13th.
